Itraconazole is the non-proprietary name for a powerful antifungal medication. More commonly patients buy itraconazole under it's more widely-recognized names of Sporanox or Orungal.

This is a well-tolerated medication, but it is also potent and should not be used frivolously. Side effects are similar to those experienced with other antifungals. It is suitable for patients age 18 and up; parents should not buy itraconazole for younger patients without doctor permission and involvement in treatment.

Itraconazole Uses

Like all antifungals, itraconazole is useful only against certain species of fungus. Ideally the type of fungus infecting a patient will be identified prior to itraconazole use; the medication will provide little benefit used against non-susceptible fungi.

The majority of patients buy itraconazole to treat:

• Aspergillosis, caused by fungi of the genus Aspergillus
• Blastomycosis, caused by Blastomyces dermatitidis
• Coccidioidomycosis, caused by Coccidioides immitis or Coccidioides posadasii
• Histoplasmosis, caused by Histoplasma capsulatum
• Paracoccidioidomycosis, caused by Paracoccidioides brasiliensis
• Oral or esophageal candidiasis, caused by fungi of the genus Candida
• Onychomycosis, infection of toenails or fingernails from Candida or other fungi

Itraconazole is available in three formulations:

• Oral solution is primarily used to treat oral or esophageal candidiasis (thrush). Taken on an empty stomach. Not interchangeable with tablets or capsules.
• Tablets are used only for treating toenail fungus. Taken with a meal. Not interchangeable with oral solution.
• Capsules are the most common and versatile formulation, used for everything except oral or esophageal candidiasis. Taken with a meal. Not interchangeable with oral solution.

Note that it's no longer possible to buy itraconazole oral solution in the USA. It is available elsewhere, but the majority of patients seeking to treat oral thrush will find it's easier and less costly to just find an alternative product, of which there are many.

Eradicating Fungi with Itraconazole

Treatment with this product can be quite variable with dosage and duration of treatment depending on a number of variables, including---but not limited to---the type and severity of the infection. Fungal infections can be difficult to detect and may go untreated for months.

Fingernail and toenail infections are among the more straightforward to treat.

Fingernails:

• Take 200 to 400 mg per day for one week.
• Stop taking the medication for three weeks.
• Take 200 to 400 mg per day for another week.

Toenails:

• Take 200 mg once per day for twelve consecutive weeks.

Other infections can have tremendous variation and it's inadvisable to attempt treatment without the involvement of a doctor. Rough guidelines are:

• Start with 200 mg per day.
• In more severe infections this may be increased to 400 mg per day.
• In life-threatening cases, 600 mg per day may be given for the first three days, before being reduced to 200 or 400 mg for the remainder of treatment.
• Treatment should continue for three months to one year, sometimes longer, until screens for the fungus show it has been eradicated.

Treatment should continue until the infection is cleared up; failure to complete treatment, in addition to leaving the patient vulnerable to reinfection, contributes to antifungal resistance.

Patients taking itraconazole will likely have regular screenings to monitor for developing side effects. Patients should be alert for symptoms of liver impairment, including jaundice, pale stools, abdominal pain (especially if it radiates to the back), and severe fatigue.

Itraconazole can interact with other medications, sometimes severely. Check with a doctor or pharmacist before mixing treatments while taking Sporanox.
